Please fill these forms out as completely as possible and bring them to your first appointment. If you prefer to fill out forms in the office, please arrive 15 minutes prior to your appointment time.

It is always best for you to check with your insurance company to verify your medical coverage. Remember that it is ultimately the patient’s responsibility to know your insurance benefits. Any fees your insurance does not cover, you will be responsible for. We accept most insurances, to ensure the best eye care possible please contact our office for more information.

You may want to bring your sunglasses with you to your appointment. As a part of your examination, your pupils may be dilated. With dilation, most people experience light sensitivity and difficulty with close focusing (such as reading). Some individuals also have difficulty seeing in the distance. The effects of the dilation can last up to several hours.
